Document workplace accidents and near misses to maintain safety records and identify hazards.

Reporting an Accident

Staff should report accidents immediately using the mobile app:

  • Tap "Accident" from the Quick Report section on Home

  • Describe what happened

  • Record who was involved

  • Note any injuries

  • Take photos of the scene if safe to do so

  • Submit the report

Near Miss Reports

Near misses are incidents that could have caused harm but didn't. Reporting these helps prevent future accidents. Use the same Accident report type and note that no injury occurred.

Manager Review

Managers receive immediate notification of accidents. Review reports in the Admin Panel under Accidents & Near Misses. Document any follow-up actions, investigations, and preventive measures taken.

RIDDOR Reporting

Some accidents must be reported to the Health and Safety Executive under RIDDOR. Paddl flags potentially reportable incidents for your review.
